A case of critical Dyspnea of Mrs Mithelesh Pal, a 50-year-old woman who was operated to relieve from the excruciating struggle to breathe was successfully handled by Chandan Hospital team. Her breathing issue began about a month ago and with time it was becoming very unbearable for her. When she consulted Dr Vishal Srivastava, Chief Consultant and Dr Shraddha, Sr. Consultant,Department of Cardiothoracic and Vascular Surgery, Chandan Hospital, she got to know about her condition of Dyspnea. In this condition shortness of breath can vary from mild and brief to severe and long-lasting. Dyspnea patients, in general, suffer from difficulty in breathing, also it is at times termed as air hunger.
Her symptoms were of a typical Dyspnea patient such as coughing, wheezing, tightness in the chest, laboured breathing, shortness of breath after exertion or due to a medical condition, heart palpitations, feeling smothered or suffocated as a result of breathing difficulties and rapid and shallow breathing. After a few tests it was diagnosed that she had Pyopneumothorax with bronchopleural fistula (BPF) in her left lung making her situation grave. Bronchopleural fistula is an unusual passageway or a sinus tract that emerges between the huge airways in the lungs. It is a grave complexity usually caused through lung cancer surgery but most times it can also occur due to radiation, infection or after chemotherapy.Doctor Vishal & doctor Shraddha without a delay advised for the patients to get admitted on the same day and suggested to go for decortication of the left lung with the closure of BPF (bronchopleural fistula) for instant relief.
Decortication is a procedure through which a restrictive layer (of fibrous tissue) is removed surgically overlying chest wall, lung, and diaphragm. Its basic purpose is to eliminate the layer permitting the lung reexpansion. After the removal of the peel, the compliance returns to the chest wall. The symptoms of patients will rapidly improve after the removal. The lung functions normally by a subsequent deflation and expansion.
Mrs Mithelesh was operated the next day after the consultation to relieve her of the ordeal she was suffering from. The operation was successful, and she was relieved within three days after the surgery with visible improvements in her health.
I was in a lot of distress as I had to fight for my every breath. It was very disturbing as I didn’t know what was causing it.
